If you regularly sleep six hours or less a night in your 50s and 60s, your risk of developing dementia later in life goes up by roughly 30 percent. That number comes from a 25-year study of nearly 8,000 people, and it is one of the strongest signals we have that sleep is not just about feeling rested. It is about protecting your brain for decades to come.
As a sleep doctor, I get this question often. People want to know if a few bad nights matter, or if this is only a problem for people who already have a sleep disorder. The honest answer is that both short-term and long-term sleep loss affect the brain, but the biggest risk comes from years of chronically short or poor-quality sleep, especially in midlife.
Here is what the evidence actually says, and what you can do about it.
Your brain cleans itself while you sleep
During deep sleep, also called slow wave sleep, your brain runs a cleanup system called the glymphatic system. Think of it as a drain that opens at night and flushes out metabolic waste that builds up while you are awake. One of the proteins it clears is beta-amyloid, the same protein that clumps together to form plaques in the brains of people with Alzheimer’s disease.
In 2025, researchers ran a randomized crossover trial where the same participants underwent a night of normal sleep and a night of sleep deprivation, in random order. The results were clear. Normal sleep allowed the brain to clear more amyloid and tau, the two proteins most closely tied to Alzheimer’s disease. Sleep deprivation blunted that clearance. This is one of the few studies in this field that used a randomized design, which gives it more weight than the observational studies most sleep and dementia research relies on.
A separate study from the University of California, Berkeley, found that older adults with less deep sleep showed faster buildup of amyloid in the brain over time. In people who already had similar amounts of amyloid, those who got more deep sleep still performed better on memory tests. Deep sleep appears to protect memory even when amyloid is already present.
What large population studies show
The strongest long-term evidence comes from the Whitehall II study, published in Nature Communications. Researchers followed nearly 8,000 British adults for 25 years, tracking their sleep duration starting at age 50. People who consistently slept six hours or less a night had about a 30 percent higher risk of developing dementia, even after accounting for heart disease, diabetes, depression, and other health factors.
Other recent findings back this up:
- A 2025 UK Biobank study of over 88,000 adults found that people who slept less during the week were more likely to develop dementia, though catching up on sleep over the weekend appeared to lower that added risk to some degree.
- A national U.S. study following adults 65 and older for 10 years found that sleeping only 5 hours a night was linked to a 30 percent higher risk of dementia compared with adequate sleep.
- A 2025 analysis estimated that insomnia contributes to nearly 13 percent of dementia cases among U.S. adults 65 and older, which translates to hundreds of thousands of cases nationwide.
- Untreated sleep apnea and habitual snoring have also been linked to higher dementia risk in large cohort studies, likely because repeated drops in oxygen and fragmented sleep both interfere with the brain’s overnight repair process.
None of these studies prove that poor sleep directly causes dementia on its own. Sleep problems can also be an early symptom of changes happening in the brain years before diagnosis. But the pattern is consistent across different countries, age groups, and study designs, and the biology gives us a believable explanation for why the link exists.
What this means for you
You do not need perfect sleep every night to protect your brain. What matters most is your average pattern over months and years, especially during midlife. Here is where to focus.
- Protect 7 to 8 hours of sleep opportunity each night. This is the range linked to the lowest dementia risk in most large studies.
- Keep a consistent sleep and wake time, even on weekends. Irregular schedules disrupt the deep sleep stages where brain cleanup happens.
- Get evaluated if you snore loudly, gasp during sleep, or feel exhausted despite a full night in bed. These are signs of sleep apnea, which is treatable and linked to dementia risk when left untreated.
- Limit alcohol in the evening. Alcohol reduces deep sleep even if it helps you fall asleep faster.
- Treat insomnia early rather than living with it. Cognitive behavioral therapy for insomnia is the first-line treatment and works better long term than sleep medication for most people.
- Take midlife sleep seriously, not just sleep in your 70s and 80s. The Whitehall study found the strongest risk signal at ages 50 and 60, well before dementia symptoms would ever appear.
When to talk to a doctor
If you have struggled with poor sleep for more than a few months, or if a partner has noticed pauses in your breathing at night, it is worth getting checked. Sleep apnea, chronic insomnia, and restless legs syndrome are all treatable, and treating them may do more than help you feel rested. It may help protect your brain for the long run.
